Please provide our office with your most current insurance information. All charges incurred will be filed to your insurance carrier. While we participate with most major health insurance companies, check with us when making an appointment regarding our participation status with your insurer.

Some insurance companies require that you have a referral for your appointment before you visit with the Women's Health Services physicians. Such referrals are sometimes needed for procedures done outside our office or for certain extended services in our office. Please determine in advance if such referrals are required by your insurance company.

While we will file all of your charges to your insurance carrier, please keep in mind that our agreement is with you, and you are responsible to pay all co-payments and/or deductibles as required at the time of service.
